Department: Office of the Dean, Peer Mentored Care Collaborative

Job Title: Incarcerated Services Project Coordinator

Position #00821672 – Requisition #33349

Job Summary:

The Peer Mentored Care Collaborative (PMCC), a business unit within the University of Colorado School of Medicine, is seeking an Incarcerated Services Project Coordinator (or equivalent). This position will coordinate innovative public health projects for individuals currently incarcerated and individuals released from prison and jails. Specifically, this role will serve as the primary lead in the design and implementation of a Colorado Peer Education Program (CoPEP) with ECHO Colorado.

CoPEP, an evidence-based model initially implemented in New Mexico, is a peer education initiative designed for implementation in Colorado correctional settings. The primary objectives of CoPEP include reducing Hepatitis C and HIV transmission, increasing testing and treatment, and fostering job readiness for the peers participating in the program. This initiative is a collaborative effort involving the Colorado Department of Corrections (CDOC), the Colorado Department of Public Health and the Environment, and the University of Colorado School of Medicine. In addition to coordination of COPEP, this position will coordinate operations with other programs on campus, at the discretion of the workflow of the PMCC PEP project.

A key partner for this position will be WORTH, a non-clinical program focusing on providing navigational and peer support to people incarcerated in a county jail. This program aims to help those transitioning back to the community feel safe and confident to connect to medical care when they are released. Due to this role's collaborative nature, this position requires excellent communication skills and the ability to effectively engage with diverse teams and community partners and drive projects to successful outcomes.

Key Responsibilities:

  • Planning, preparing, and facilitating a week-long train-the-trainer workshop to train incarcerated individuals to become health educators.
  • Oversee the professional development of incarcerated individuals, which includes collaborating with subject matter experts to create and update training content.
  • Traveling to the selected CDOC prison sites and providing continuous feedback and updated health education information for peer educators. Note: Travel Expenses (including mileage) are reimbursed according to University Fiscal Policy and methods of travel will be delineated by project leadership.
  • Collaborates with the staff and liaisons from the selected CDOC prison sites throughout Colorado, which includes coordinating dates and security clearances for training and traveling to CDOC to interview potential trainees.
  • Orders and organizes all supplies needed for training which include documents, curriculum, snacks, hotel reservations, rental car reservations and organizing other facilitators for the training.
  • Organizes and coordinates monthly video conference ECHO sessions with ECHO staff, content experts, and/or other peer educators.
  • Coordinates the collection, compilation, and analysis of activity data for the projects; develops, writes, edits, and presents comprehensive statistical and narrative program reports and evaluations.
  • Assist in planning, developing, and writing grant reports and funding opportunities for project activities.
  • Coordinates and manages monthly CoPEP team meetings with multiple partners and stakeholders.
  • Collaborates with subject matter experts and WORTH staff to develop comprehensive project processes and policies, curriculum, tools, outcomes, and timelines, and monitor progress against established metrics.
  • Lead the planning, execution, and delivery of the projects, ensuring prioritization of the needs of the previously incarcerated community, adherence to evidence-based best practices, and compliance with all University rules and regulations.
  • Organize all documentation of data relevant to the projects, to ensure it is both accurate and timely.
  • Organize resources, including personnel, equipment, and materials, to ensure optimal project execution and delivery.

Why Join Us:

The Peer Mentored Care Collaborative (PMCC) is home to two innovative and highly regarded programs: ECHO Colorado (Extension for Community Health Outcomes) and eConsults. PMCC aims to improve the quality of care and patient experiences while reducing overall costs.

PMCC leverages partnerships with organizations and communities across the state of Colorado to meet rural providers where they are to improve each domain of the quadruple aim: increasing access to care, reducing costs and unnecessary care, improving patient outcomes, and improving patient and provider satisfaction with care.

Through the data collected from PMCC program utilization, PMCC can anticipate and identify health trends in local communities, share and exchange vital resources and best practices, shape patterns of care to ensure high-quality care that costs less, and keep the patient and the local provider at the core of care delivery.
